Alzheimer's Disease Initiative - Specialized Supportive Services (ADI-SSS) Project Financed Solely by 2016 Prevention and Public Health Funds (PPHF 2016)
The purpose of the ADI-SSS program is to provide grants to public and private entities that are working within existing, dementia-capable, long term services and supports systems and are committed to serving populations with the most need and living with or at risk of developing Alzheimer’s
disease or a related dementia (ADRD) and their caregivers.
Successful applicants will propose services designed expand their existing demential-capable service system to address the needs of three out of four identified service gaps identified in the Funding Opportunity Announcement.
The grantees benefit from targeted technical assistance provided by the National Alzheimer’s and Dementia Resource Center which is funded through a contract awarded under the Alzheimer’s Disease Supportive Services Program (ADSSP).

This competition is limited to organizations working within an existing, dementia-capable, long-term services and supports system and those that have not already received AL grants.
Entities not operating within existing dementia systems and those that received awards as a result of the following funding opportunity announcements in 2014 and 2015 (HHS-2014-ACL-AOA-AL-0090 and HHS-2015-ACL-AOA-AL-0104) are excluded from eligibility.
Successful applicants of the 2014 and 2015 opportunities include Alzheimer's Association of Greater Michigan, Alzheimer's Community Cares, Alzheimer's Greater Los Angeles, Alzheimer's San Diego, District of Columbia Office on Aging, Delaware Department of Health and Social Services, Family Services of Westchester, Florida Atlantic University, Georgia Southwestern University, Greenville Health System, Jewish Family & Children's Services of Sarasota, FL, Lutheran Services of Puerto Rico, Minnesota Board on Aging, Neighborhood Centers, Inc., Orange County Department on Aging, Seven Hills of Rhode Island, Southern Maine AAA, United Way of Tarrant County, University of Hawaii, Virginia Department for Aging and Rehabilitative Services, and Wisconsin Department of Health Services.
Individuals and foreign entities are not eligible to compete for, or receive, awards made under this announcement.
