The purpose of NOFO CDC-RFA-DP15-1514 is to improve the stroke systems of care through data collection and identifying areas to improve health outcomes and guide quality improvement efforts at various stages of care.

Specifically, the activities will include:
Continuing the collection of pre, in and post hospital data; Maintaining the established data management system to collect and link pre, in and post hospital data and ensure data quality; Analyzing and using the data collected to improve care and transitions of care for stroke patients; Submitting the data to CDC according to CDC guidelines; Coordinating the implementation of quality improvement activities with emergency management services and hospital partners; Coordinating and promoting public stroke messaging; Maintaining a steering committee for the program to advise and guide the work of the program; and Continuing partnerships with state and/or regional Emergency Management Service Directors.
